Seniors Social Prescribing Program
Hours of Service: Flexible throughout the week
Purpose Statement: To connect seniors to community programs, services and resources that will help them thrive, and remain connected and independent for as long as possible.
About Program: The Seniors Social Prescribing Program is a pathway from your doctor or nurse practitioner’s office to our Seniors Community Connector for seniors who could use a little help connecting to their community or learning how to take care of their health and wellbeing. Social Prescribing works alongside clinical treatments to address people’s needs in a holistic way.
Social prescriptions may include:
- physical activities
- nutrition/food security programs
- social programs and services in the community
- caregiver programs
Who Qualifies/Eligibility: Seniors in Maple Ridge and Pitt Meadows who are:
- experiencing poor health (physical and emotional)
- marginalized
- socially vulnerable
- making frequent trips to ER or primary care
Cost: Free
