Community Living Support Program

Providing community and home-based services, which builds upon the strengths of individuals living with mental health issues, and supports them in maintaining their independence and in realizing their potential.

Two outreach programs to adults are offered:

bulletCommunity Living Support Program (CLS)
bulletSemi-Independent Living Program (SIL)

Community Living Support Program

The Community Living Support Program provides community support to individuals living with mental health issues.  Clients are supported by outreach workers to achieve increased independence in managing their daily activities through the provision of education, training, and by increasing clients' stability in their community.

Semi-Independent Living Program

The Semi-independent Living Program is a supported housing program that enables individuals with mental health issues to live independently in safe, affordable, self-contained living units.  Clients are supported by outreach workers to achieve increased independence in managing their daily living activities and to increase client's stability within their community.

Individuals in the SIL Program are eligible for a B.C. Housing Rent Subsidy as well as a Utility Subsidy.

Eligibility For the CLS/SIL Programs

bulletDiagnosed with mental health issues
bullet19 years and older
bulletReside in Maple Ridge or Pitt Meadows
bulletRegistered client of the Maple Ridge Mental Health Center
bulletIn receipt of income assistance or other source of income
bulletDo not require 24 hour staffed residential care
bulletAre in need of support services in order to live in a self-contained apartment within the community

Services Offered

bulletOngoing, flexible support of a CLS worker
bulletIndividuals may receive assistance and support to gain or improve upon the following active daily living skills:
bulletPersonal care
bulletHousehold management
bulletNutrition
bulletMoney management
bulletPersonal effectiveness
bulletProblem solving
bulletCommunity integration
bulletHousing support (SIL Program only)
bulletSomeone to talk to about independent living issues
bulletIncreased confidence in daily living skills i.e. budgeting, shopping, decision making
bulletAdvocacy and liaison with community resources
bulletCrisis management (with Mental Health team)

Program Expectations

Client, CLS Worker and Primary Mental Health Therapist will meet to write Individual Service Plan Agreement (ISPA) based on the client's individual needs.  The ISPA is client directed.

Client's to meet CLS worker during the hours agreed upon in the initial ISPA

Programs emphasize a team approach among the client, CLS worker and Primary Mental Health Therapist

All individuals receiving support services from the CLS/SIL Programs maintain the right to refuse service

Clients may choose to withdraw from a Program voluntarily or may discontinue the Program upon achievement of Individual Service Plan goals and no longer require any CLS or SIL support services

To Access Service

Adults living with mental health issues can access our services with a referral from the Maple Ridge Mental Health Center at 604-476-7165.

Service Hours

Programs are provided at no cost and are available from Monday to Friday 9:00am - 4:00pm and by appointment only on Saturday.  For more information about our Programs and other Mental Health Services including the Rainbow Club call 604-467-0544.

Funding

The Community Living Support Programs are funded by the Ministry of Health Services.
