Continuous Quality Improvement

Philosophy
Maple Ridge/Pitt Meadows Community Services is committed to maintaining and continuously improving the quality of the services we provide in our community.  While ensuring that our service is of exceptional quality and is part of the day-to-day culture in this agency, we have also established formal procedures and policies to manage and monitor our quality assurance activities.

History and Role of the CQI Committee
The Continuous Quality Improvement committee was struck and met for the first time in July of 2001.  The committee is comprised of line staff, volunteers, supervisors, management and the Clinical Service Consultant.  Initially, the committee reviewed both the existing Quality Improvement activities within the agency and those required by the Council on Accreditation.  Following this review, a set of tasks was determined, a time-line set and the work began.

The Clinical Consultant chaired the committee in the initial stages.  However, a Continuous Quality Improvement Coordinator was selected from among the agency staff and that person took on the task of coordinating CQI activities along with a co-coordinator from the management team in April 2002.

CQI Committee
The CQI committee meets monthly to oversee and coordinate CQI activities, and to facilitate the transmission of information to stakeholders and the community.  The CQI committee also coordinates the quarterly file reviews and facilitates the findings throughout the CQI process.  The committee creates a quarterly summary that is forwarded to the CQI Advisory Committees, personnel and the Board of Directors.  Minutes are taken and distributed to stakeholders.

All agency CQI activities are formally reviewed, summarized, and provided to our stakeholders and governing body on a quarterly basis: March, June, September, and December (* The months following the quarterly CQI month).

Stakeholder Participation
Maple Ridge/Pitt Meadows Community Services actively requests the participation from stakeholders in the agency’s Continuous Quality Improvement process, as well as in the day-to-day activities of the agency.  This is achieved throughout all levels of the agency as outlined below.

Maple Ridge/Pitt Meadows Community Services encourages participation from clients at the following times:

  • Annual client satisfaction survey
  • Client intake and service planning
  • Explanation of complaint procedures at intake
  • Client service termination
  • Integrated Case Management meetings
  • CQI Advisory Committees

Maple Ridge/Pitt Meadows Community Services actively requests participation of clients and community members/partners during the following events:

  • Community meetings such as C.Y.F.N., Homelessness project, Chamber of Commerce, Greg Moore Youth Centre meetings, Mental Health Peer Support meetings etc.
  • Annual General Meetings, Agency brochure, Agency surveys
  • Community events such as Walk-A-Mile, Seniors Week
  • Annual Community Satisfaction Survey
  • Service Stream CQI Advisory Committees

Maple Ridge/Pitt Meadows Community Services encourages the participation of personnel in the Organization’s CQI process during the following events:

  • Agency’s Risk and Remedy box
  • Organization’s Annual Satisfaction Survey
  • Service Stream CQI Advisory Committees
  • Employee/Volunteer Orientation Handbook
  • Personnel Exit interviews
  • Annual performance reviews
  • Internal staff committees (Training and Development, Community Relations, Occupation Health and Safety, CQI Committee, File Review)
  • Contributions to Organization’s newsletter
  • Explanation of agency’s grievance procedure at orientation
